Home health care services cater to a wide range of conditions, including congestive heart failure, diabetes, chronic obstructive pulmonary disease (COPD), recovery from total knee or hip procedures, wounds from pressure injuries, and more. Patients may be eligible for home health if they need the following basic criteria:
- Your doctor prescribes home health care for you.
- You need either skilled nursing care or therapy (physical/occupational/speech therapy) on an intermittent basis.
- Your condition limits your ability to leave home, rendering you “homebound.” You're considered homebound if you're unable to leave your home or must go through considerable effort when doing so, or if leaving home is not recommended because of your medical condition.
Our Medical Social Worker provides emotional support, connects patients with resources, and advocates for their needs during illness or recovery.

The first home health visit is essential to your home health journey. Our team will come to your home to discuss your personal health goals and ask questions about your health needs. This is also an opportunity to share your preferences and concerns so that our team can tailor your experience so that you have a safe, supportive, and professional environment in your home.
