Who is eligible for Home Health?

Home health care services cater to a wide range of conditions, including congestive heart failure, diabetes, chronic obstructive pulmonary disease (COPD), recovery from total knee or hip procedures, wounds from pressure injuries, and more. Patients may be eligible for home health if they need the following basic criteria:

  • Your doctor prescribes home health care for you.
  • You need either skilled nursing care or therapy (physical/occupational/speech therapy) on an intermittent basis.
  • Your condition limits your ability to leave home, rendering you “homebound.” You're considered homebound if you're unable to leave your home or must go through considerable effort when doing so, or if leaving home is not recommended because of your medical condition.
What are the benefits of Home Health?

Our Medical Social Worker provides emotional support, connects patients with resources, and advocates for their needs during illness or recovery.

  • Keeps patients comfortable at home, where they typically recover faster with fewer complications
  • Reduces avoidable trips to the hospital
  • Supports caregivers and loved ones
  • Uses evidence-based clinical practices so you receive high-quality care backed by science
  • Offers an interdisciplinary approach that gives you access to a diverse team of healthcare professionals
  • Provides personalized care planning tailored to your needs
Who pays for Home Health services?

We will get insurance authorization through your insurance to treat your needs. The office will notify you before services begin if there are any copays or other fees. In most cases, primary Medicare policies cover our services 100%.

What can I expect on the first Home Health visit?

The first home health visit is essential to your home health journey. Our team will come to your home to discuss your personal health goals and ask questions about your health needs. This is also an opportunity to share your preferences and concerns so that our team can tailor your experience so that you have a safe, supportive, and professional environment in your home.
