Refer a Patient
Anyone can call and request an informational visit about palliative or hospice care. The patient, his/her physician, a family member or a friend may initiate services by calling our office at 540.776.0198. Once a referral has been made, our team schedules a time to meet the patient and family to answer questions and provide further information.
Following this initial visit, a nurse meets with the patient and family to assess their needs and develop a personalized care plan. All patients remain under the care of an attending physician. Patients also receive additional support through a hospice medical director, RN, medical social worker, aide, volunteers and chaplain, if desired. The most important part of the team is the patient and his/her family and caregivers who determine the care plan.
Patients may live at home alone, reside with a caregiver or live in a nursing home, hospital or assisted-living adult care facility. Good Sam also has a “Living Alone” program that is specifically designed to help ensure safety and allow a greater level of independence for patients without a caregiver.
Hospice care is funded through Medicare, Medicaid and most private insurance plans. The many generous donations we receive throughout the year also allow us to provide care regardless of a patient’s ability to pay.
Palliative care is covered by most insurance plans, including Medicare and Medicaid. A customary office co-pay may apply. If cost is a concern, a social worker from the palliative care team can help.
